The Circle of Faith Hospice team offers a holistic approach to Hospice Care with a focus on the quality of life for those facing a life-limiting illness. By focusing on the quality of life, we help our patients to make the most of the time that they have left to live and to live it well. We understand that this is a season that is full of emotion and that Hospice Care is often met with fear and confusion. We aim to minimize your concerns and to eliminate the uncertainty associated with the medical and non-medical care that one receives under their treatment Care Plan.

Hospice is not a place

Hospice Care is not a place, and Circle of Faith Hospice delivers care and support to wherever it is that you call home. We visit patients in their homes, in the home of a loved one, at assisted living facilities, nursing homes, and hospitals.  We believe that caring for our patients in the environment where they feel comfortable, safe, and secure is optimal. Studies indicate that patients on Hospice Care that are able to reside in their own homes experience less irritability and anxiety, allowing the patient to rest easier than if they hospitalized.

If you've opted to experience your final days savoring the life that you have left and celebrating the life that you've lived rather than choosing to seek treatments that will only prolong your life for a short period of time, then we would like to help. We Support your Support Team

We are not just here to serve the patient, we are here to educate and support those that have always been there for you and that strive to give you the emotional and physical support and care that you require. We understand that this time can be wrought with a sea of emotion and that caregivers are prone to neglect to care for themselves or becoming overwhelmed with trying to balance their own responsibilities with caring for you.

The majority of the caregivers that we encounter tend to experience a sense of relief once the patient's hospice care team is assembled. By reassuring them that they are not alone, answering questions that have been weighing heavily on their minds, and relieving them of "playing nurse" will allow them to refocus their energy on spending time with their loved one. Returning to the role of wife, mother, husband, father, daughter, son, sister, or brother is often perceived as a gift.

Respite Care

Another important benefit of Hospice Care for caregivers is providing relief in the form of Respite Care. By providing Respite Care, this allows them to attend a special event or appointment, or just to get some time alone.

Bereavement and Grief Support

Experiencing the loss of a loved one is never easy, and we understand that the time following the passing of their loved one is a difficult road to travel. Circle of Faith Hospice provides assistance in the form of peer counseling, group support, and the opportunity to participate in a celebration of life ceremony to honor their loved one.

Hospice is not about Giving Up
