Can Palliative or Hospice Care benefit my patient?

Welcome to Circle of Faith Hospice, we are glad that you're here. If you have a patient that is appropriate for a Palliative or Hospice Care referral then we urge you to contact us to determine how we can assist you and your team to relieve symptoms due to a serious illness.

By collaborating with your team, you remain informed and updated. We respect the close relationship that physicians, nurses, and other staff members develop with their patients, and we will strive to add value to the services that you offer.

What is in it for you?

Circle of Faith Hospice is a valuable resource when it comes to medication management, symptom relief, social and spiritual support, and therapies as needed. We strive to decrease hospital visits, including emergency care, by taking a proactive approach. You may choose to remain the Primary Care Physician while the patient is on palliative or hospice care, or you also have the option to transfer the care of the patient to our in-house Hospice Medical Director. Circle of AFaith Staff will maintain communication with you, your patients, and their families, providing skilled registered nurse support 24/7, and ongoing palliative or hospice educational guidance.

When should Hospice be considered?

The hospice benefit covers all expenses related to the terminal illness, including medication, nursing care, and equipment. Hospice should be considered when a patient has End stage heart failure, Advanced Dementia, Activity-limiting lung disease, metastatic cancer, HIV, ALS, End stage Renal disease, or another illness where a cure is not the achievable goal.

When should a patient be referred to Palliative or Hospice Care?

Timely referrals are beneficial to all parties, the patient, their loved ones, caregivers, physicians, and the hospice agency. According to the AJMC Guidelines, Palliative care should be introduced early and in parallel to their active treatment plan.
