So, you might be thinking, what happens next. If your health care coverage is with Medicare Part A, Medicare Advantage Plan, Medicaid, or other health care plans, then in most cases your health benefit plan will cover hospice care or palliative care.

When a referral is received requesting palliative or hospice care from your physician's office, a member of our clinical staff will contact you to schedule your intake assessment. Once the assessment is complete, we will generate a custom care treatment plan that will be designed around your physical, emotional, and spiritual needs. Your care treatment plan will also document other details about your lifestyle, your values, and your concerns. By documenting these details, we ensure that your wishes are implemented.
